Give us a call at (606) 453-3901 to set up your appointment. Walk-ins are welcome when possible, but appointments are preferred.

Please bring a valid ID, your insurance card, and a list of current medications and any relevant medical history.

Plan to arrive 15 minutes before your appointment to complete any new patient paperwork and get settled.
